AI that gives doctors their time back

Doctors spend up to 68% of their time with admin work instead of looking at their patients. Sikia AI builds AI agents to supercharge doctors — so they can focus on people, not paperwork.

Our first product an AI assistant for documentation and billing is already live in hospitals and practices across Germany, documenting thousands of patients every week. The product works, doctors love it, and we’re scaling fast. Now we’re hiring the founding engineer to take us from here to the next level.

The role

You’ll build alongside the CTO and own critical systems end-to-end. Your code runs in real hospitals within weeks — you’ll talk to the doctors using it, then make it better. Maximum ownership, real founding equity (VSOP), a company built 0 to 1.

What you’ll own

  • The Python backend powering real-time medical documentation in production

  • Live transcription on real medical conversations — streaming audio, multiple STT providers, quality monitoring and fallback

  • Our applied-AI core: LLM evaluation pipelines, prompt optimization, multi-provider routing and fallback chains, and the cost/latency tradeoffs that come with running AI at scale

  • Multi-cloud production infrastructure keeping all of this reliable in clinical settings

  • Greenfield projects defining what the AI-doctor becomes next

Already shipped systems like this? You’ll start senior and own the core immediately. Strong engineer who hasn’t yet, but is hungry to? We’ll get you there in months, working directly with the CTO.

Full-stack when it counts — the work leans backend and AI, but the product is Flutter (Windows + Web) and you’ll jump in when it needs you. Small team: we go where the problem is.

The stack

  • Backend & AI: Python, FastAPI, real-time WebSocket audio pipelines; a multi-provider LLM layer with our own routing, caching and fallback

  • Real-time transcription: streaming STT across several providers with health-aware failover

  • Eval & observability: an offline LLM-as-judge evaluation harness, plus production tracing and error monitoring

  • Data: Supabase / Postgres

  • Frontend: Dart / Flutter (Windows desktop + Web)

  • Infra: multi-cloud, Docker, CI/CD

  • Tooling: we build with the modern AI toolchain every day — Claude Code, Codex, Cursor

You'll fit if you

  • Are genuinely strong in Python — it's our core language — with at least three years of coding experience

  • Build things, not just tickets — you prototype on weekends, ship side projects, and want to put your mark on a product that matters

  • Have owned real systems end-to-end and shipped things people depend on

  • Have high agency — you see a problem, make the case, ship the fix

  • Build with modern AI tools and want to go deep on applied AI (eval, prompting, routing, real-time pipelines) — whether you've done it already or are hungry to

  • Think like a co-founder: you challenge our approach, spot opportunities, and push Sikia to the next level

  • Have empathy for the doctors and patients we build for

  • Understand conversational German

Nice to have: Dart/Flutter (esp. Windows desktop), a creative product eye, a CS/MINT/medical background — or self-taught with serious shipped projects.
